ADDI Medical has signed a regional agreement with Region Värmland for the use of HOPE for heart failure. The region is thereby continuing to use HOPE for heart failure, an AI-supported care process with the patient as an active partner via an individual self-care plan.

In connection with the agreement, ADDI Medical has delivered an extended dashboard that gives healthcare providers a clearer overview of patients. The dashboard displays graphs, risk-classified views and tables containing statistically processed values, individual measurement values and historical form responses. This provides healthcare staff with cohesive support to see which patients require attention and to monitor developments over time.

HOPE for heart failure is part of the digital platform HOPE and provides healthcare services with integrated support for home monitoring, follow-up, risk classification and communication for patients with heart failure. The service is based on an AI-supported care process where healthcare professionals can plan, schedule and follow up on the patient's self-care. The care process includes clinical decision support, an individual self-care plan, reminders, notifications, risk classification and secure email as support for communication.

The patient receives an individual self-care plan via the HOPE App and is guided step by step through measurements, screening forms and follow-up. Blood pressure, pulse and weight are measured at home via connected devices and transmitted automatically to HOPE via 4G. The patient does not need to enter these values manually.

At the start, the patient completes a learning week with closer measurements and screening. Following the learning week, the patient's self-care plan is established jointly with healthcare services based on the patient's values, needs and the care prescribed. The plan can subsequently be adapted over time.
